This website is currently a non-public preview. The inquiry form validates entries only within the browser and is intentionally not connected to a submission service.
Callback requests
When callback delivery is enabled, the form collects an adult’s name and one selected callback method. It provides no free-text field and instructs visitors not to submit information about a child, diagnosis, symptoms, therapy history, or other health information. The website transmits the request to the practice through its hosting provider and approved Google Workspace account so the practice can respond. The website does not store callback requests in a database; the resulting notification remains in the practice mailbox according to the practice’s retention procedures.
Google scheduling
The contact page embeds a public Google Calendar booking page, which loads content from Google when you visit the page. A separate link opens the same booking page in a new tab. The website does not prefill or transfer form values to Google. Google independently requires attendee name and email for bookings; visitors are instructed not to enter child or health information. The practice must approve its Google Workspace configuration, account terms, booking-form fields, access controls, and privacy disclosures before launch.
Health information
This website and its public scheduling page are not patient portals. Do not provide a child’s name, diagnosis, symptoms, therapy history, or other health information through either channel. Final language describing approved intake, protected health information, third-party providers, retention, and individual rights must be reviewed before online intake is enabled.
